Monophasic: same dose of estrogen & progesterone; Biphasic: vary the dose of progestin; Triphasic: vary dose of estrogen, progesterone, both Combined Contraceptives Starting Methods First day start: the first pill taken on the first day of menstrual cycle, no backup method needed Sunday Start: First fill taken on the Sunday following the start of menses, backup method needed for first 7 days, menses occur only during the week Quick or “Same Day” start: First pill taken on the day of the office visit, backup method for the first 7 days Instructions for missed OC Topical patch Ortho Evra patch releases 20 mcg of estrogen & 150 mcg of norelgestromin: apply once a wk on the first day of menses (starts other days if backup method used) for 3 wk then 1 wk off (bleeding occur); place on abd, upper torso, outer arm or buttock; High BMI (>198 lb) increase failure rates Vaginal rings NuvaRing is a soft, flexible plastic ring that releases 15 mcg of estrogen and 120 mcg of etonogestrel daily; Ring is placed in the vagina ( not requiring fitting ), left in place for 3 weeks, then 1 week off; Better cycle control and decreased breakthrough bleeding compared to OC; Lower systemic exposure to estrogen Progesterone-only Pills Progestin- only Pills Indication: when estrogen is contraindicated (effect: thickening of cervical mucus and prevention of sperm penetration), breastfeeding, history of migraine d/t lower level of hormone contained, diligent compliance of daily dosing at the same time is important (even a few hr late, backup method is recommended for the following 48 hr) ADRs: Changing bleeding pattern and breast tenderness Injectable -only Progestin s Depot medroxyprogesterone acetate (Depo-Provera) is a long-acting, lasting the effectiveness of suppressing ovulation for 12-13 wk Advantages: dosing once every 12 wk—reliable efficacy (acceptable up to 1 wk late)
