6. List and describe the tests used in the laboratory to assess the exocrine function of the pancreas Tests in chronic pancreatitis: - Secretin test measure volume; bicarbonate. A double lumen tube is positioned with the aid of fluoroscopy so that the tip of one tube sits in the duodenum and the other in the stomach to remove gastric juice. Two 10-minute basal collections of duodenal juice are obtained to stablish a baseline; secretin is then administered intravenously, and 8 more specimens are collected in 10-minutes intervals for the measurement of bicarbonate and volume. Normal, pancreatic response to secretin is to increase secretion of fluid with high concentrations of bicarbonate. - Lundh test meal trypsin in duodenal aspirates. Trypsin is analyzed in duodenal aspirates obtained after a meal; pt fasts 12hrs. a tube is inserted into the third of the duodenum once again under fluoroscopic guidance. Test meal containing: 8g corn oil, 15g casein, 40g glucose in water given. Pt is placed on his right side and the tube is allowed to drain in an iced container. Tube is allowed to drain for next 2hrs. trypsin activity is measured in the aspirate. Trypsin activity is significantly decreased in chronic pancreatitis. Test will be abnormal I 90% of pt with chronic pancreatitis and 79% with pancreatic carcinoma.
