Have the team list all of the steps in the process. Fill out the table with your team. Include the topics of failure mode, failure causes, failure effects, likelihood of occurrence, likelihood of detection, severity, risk profile number, and actions to reduce occurrence of failure. Then use the RPNs to plan action improvement measurements. Testing of interventions from the process improvement plan will include feedback from nursing staff on the new patient to staff ratios. I want to know how the staff is adjusting and do they feel that they can provide better and safer care to patients with the changed ratios. I can lookat data for patient wait times to be seen and wait time for interventions completed by staff. Completing patient interventions in a timely matter or in a quicker manner ensures better outcomes and satisfaction for patients. This data can be verified statistically by reports and numbers gathered for each patient. As a professional nurse, monitoring patient outcomes and quality of care is very important. Patient outcomes and quality of care go hand in hand. As I spoke earlier, being mindful of patient wait times plays a very important role in both of these. The longer a patient has to wait to be seen drastically affects the quality of care that they receive. If it is an emergency, seconds matter and if there is not enough staff to appropriately care for patients, then those precious seconds are lost. Losing these seconds can have a major effect of patient outcomes. As a leader it is important to know what the appropriate wait time is for patients and

Quality improvement activities would include involving staff nursing in auditing and evaluating their personal intervention times. When staff is involved in the care and their evaluation of their own care that they provide, more attention and effort is put forth. Being in nursing leadership and having involvement with the RCA and FMEA process is very important in that the nurse provides insight of the proper way that procedures should be done. The nurse leader can also provide ideas of change that would greatly affect outcomes.
