Risk Assessment Instruments The collection and analysis of information regarding an individual’s current and overall health is a health assessment (Ball et al., 2015). The provider an identify patient’s needs and complete specific assessments based on the needs or problems identified by completing a Health Risk Assessment (HRA). A functional assessment should be part of the interview and assessment, because patient has a disability and is at a higher risk of injury, and self-care deficit also taking into consideration the patient’s advanced age and ethnicity, the risk assessment should be able to address many aspects of care and daily life. A functional assessment helps to provide an understanding of an individual’s ability to perform activities of daily living (Ball et al., 2015). A proper functional assessment includes assessing mobility, home safety, ability to perform activities of daily living, house work, meal preparation, adherence to medication regimen, and management of finances. The use of health risk assessment according to Wu & Orlando (2015), will assist in making an accurate clinical decision tailored to the patient. Targeted Questions What brings you in today?
