A unifying mechanism is still unproven. 2 Etiology The causes of irritable bowel syndrome remain poorly defined, but they are being avidly researched. Up to one third of patients with irritable bowel syndrome may have altered colonic transit. Colonic muscle hyper reactivity and neural and immunologic alterations of the colon and small bowel may persist after gastroenteritis. 3 Epidemiology Population-based studies estimate the prevalence of irritable bowel syndrome at 10-20% and the incidence of irritable bowel syndrome at 1-2% per year. However, when a 2016 population-based study compared the prevalence of irritable bowel syndrome with the Rome IV criteria versus the Rome III criteria in the United States, Canada, and the UK, investigators reported a nearly 50% reduction. 4 Clinical Manifestations People may experience pain in the abdomen. Gastrointestinal symptoms include change in bowel habits, constipation, diarrhea, inability to empty bowels, indigestion, nausea, passing excessive amounts of gas, or urgent need to defecate. Also, common symptoms include anxiety, depression, discomfort, loss of appetite, or symptoms alleviated by defecation.
