It is important to encourage nurses to work with other members of the interdisciplinary
team to improve patient safety and standard of care. This can be accomplished by
developing a standardized handoff report that includes different focuses or disciplines
that are patient centered.
Within two years, seventy five percent of the nursing staff will have incorporated the
handoff reporting process into their daily processes this will meet hospital guidelines.
By the third year, all departments will be participating in the new protocols for
standardized handoff.
Further implementation of the VBP:
When there is a breakdown
in communication patient safety is compromised.
Communication barriers/breakdown account for
60% of the root causes of sentinel
