Task 2: Investigate and Communicate Workplace Incidents
ASSESSMENT INSTRUCTIONS
This task will require you to investigate and communicate at least two workplace incidents.
Workplace incidents can be one of the following:
 Workplace accident
 Workplace injuries or illness
 Environmental event (e.g. emissions, noise etc.)
 Physical security incident (e.g. break-in)
 Cybersecurity incident

 Workplace harassment or bullying
Use your workplace/organisation’s template to complete this task, or use the Generic Incident
Report template provided at the Bounce Fitness site.
To complete this task, you must:
 Investigate at least two workplace incidents by interviewing at least two identified
individuals or groups for each incident, according to organisational policies and
procedures.
Identified individuals or groups are individuals or groups who were either involved in the
incident or were witnesses to the incident.
 Apply problem-solving processes for at least two workplace incidents to:
o Identify and assess risks
o Identify risk control measures
o Determine action plans
 Prepare incident reports for at least two workplace incidents, according to organisational
policies and procedures, including:
o Description of the incidents, including description of any injuries, if applicable
o Identification and assessment of risks of similar incidents repeating
o Identification of risk control measures
o Recommendation for action plan based on the risk control measure(s) identified
 Communicate each incident report to at least two relevant stakeholders
Relevant stakeholders are individuals or groups in your workplace who you need to report
incidents to as per your organisational policies and procedures.
