Complete an incident report for the hazard that is involved in the scenario, using the template provided. You will need to create names and additional information as necessary to fill in the form. 3. Add Rose’s incident from the scenario to the data sheet and translate t hat data into a graph. Analyse the data and make notes in the space provided regarding what you will need to focus on during the safety action meeting. As part of your analysis, consider procedures that can be implemented to control risks using the hierarchy of control. 4. Conduct a safety inspection of your work area and document the inspection using the template provided. 5. Organize and conduct a safety action meeting (SAM) with your work safety team. Negotiate and schedule the meeting with your work safety team. Before the meeting, work with your group to: i. consult on analysis of the hazards identified in the scenario as well as the hazards identified in the inspection and discuss possible treatment ii. consider the costs of control measures for the hazards During the meeting, use the hazard hierarchy of control to recommend practicable treatments for the hazards. Also discuss procedures that can be implemented to control risks using the hierarchy of control and inadequacies in existing risk controls. Ensure you take minutes using the template provided, as evidence of the meeting. 6. Write a report for your health and safety representative. Using the template provided, ensure that you address the following in your report: a) describe the process you undertook to identify, analyse and consult on the risk b) refer to: • specific OHS/WHS legislation relating to the maintenance of a safe workplace • specific OHS/WHS legislation and regulations and codes of practice requiring institution and implementation of consultative practices • duty of care responsibilities of PCBU and employees • describe WHS performance with respect to identified hazards; outline any inadequacies in existing risk control measures • describe how inadequacies were monitored once they were identified
