Evidence-based Guidelines & Ethical Considerations End-of-life situations involve not only the patient but also the family. Often, family does not agree with withdrawing life-sustaining measures and may want to override a patient’s advance directive wishes when a patient is not able to verbalize their own wishes. This can be a very emotional and stressful situation for both, the providers and the family. Sometimes it may be appropriate to attempt to resuscitate a patient with advanced cancer, however, the provider must have all the facts and must make the family aware of their options, especially if an advance directive is in place. The family must also be prepared and informed about potential negative outcomes since studies have found that the survival rates to discharge following in-hospital cardiac arrest in patients with advanced cancer are lower compared to those without advance cancer (American Society of Clinical Oncology, 2017). However, if resuscitation is not appropriate and the patient does not wish life-sustaining measures the providers must educate the family on hospice care to keep the patient comfortable and to allow the family to grief and say their goodbyes.
