While some elders are unable to ambulate independently, this too prevents them from seeking the appropriate medical attention they require and decreases the prevention of further morbidities. The elderly are already a more compromised group due to a 4465-5_19
natural decrease in activity and lack of support. Therefore, the risk for developing Cardiovascular Disease or other chronic morbidities is much higher and detrimental. Thorough monitoring of risk factors that can lead to Cardiovascular Disease must be included in the health and well-being of the elder population. In order for this to be attainable, preventative care must be applied, and access to public transportation is imperative to meet to achieve preventative care. Lack of transportation to attend regular health check-ups cause in a breakdown in the healthcare system. As a result, the elder population continues to suffer from Cardiovascular Disease. According to the Center of Disease Control, (2005), “Heart Disease and cancer have been the two leading causes of death for person 65 years of age and older for the past two decades, accounting for nearly a million deaths among older persons were due to heart disease, including heart attacks and chronic ischemic heart disease.” 3. Identify two (2) Goals. For example, goals indicate where you want to go. Then, identify two to three (2-3) objectives per goal . For example, objectives should indicate how you plan to get there, or how you plan to accomplish the goal(s). Goal 1 : The Fire Department will receive less calls that refer to the primary care health needs and transportation request of the elderly population that resides in Willis, Texas. 1. Objective: Develop Senior citizen programs that offer transportation to and from scheduled medical appointments free of charge. 2. Objective: Reach out to families or support groups of the elderly who live alone, and identify those at greatest risk of developing CVD. 3. Objective: Enhance the amount of people to help the elderly with activities of daily living if family members are unable to provide assistance Goal 2: Develop a community health outreach event quarterly to allow home visits to the elderly that unable to seek medical care due to transportation or immobilization issues. 1. Objective: Implement a community program to attract assistance for local healthcare provides to provide home health calls to elders, especially those at greatest risk for CVD.
