patient in the facility. Patients often have more than one healthcare provider providing care and writing medication orders. This can result in health-care providers writing orders for similar drugs or for drugs that are contraindicated, meaning they are not supposed to be administered together. Discharge Instructions When the physician writes a discharge order for a patient, the nurse completes a discharge instruction form, which includes: When to return to the physician’s office for follow-up A list of medications and how to take them and what side effects to watch for Required modifications or restrictions of diet or activity level Signs and symptoms of worsening condition that warrant physician notification Care or treatment that the patient is to continue at home Final Nursing Responsibilities Discontinue any equipment and tubes that are to be removed prior to discharge and assess the patient’s vital signs if they have not been assessed within the last hour. Document the patient’s condition and vital signs in the nurse’s notes, along with the time the patient actually leaves and the method of transportation. Most facility insurance carriers require that patients be taken to the car in a wheelchair to reduce the risk of falls or injury. Make sure to notify housekeeping so that the room can be given terminal cleaning. This includes disinfecting the bed, furniture, bathroom, sink, and floor. The bed must be made with fresh linens and the room prepared for admission of the next patient. Patient Transfer Transfer Within the Facility The simplest transfer is the transfer of a patient from one unit to another unit within the same facility. To complete this type of transfer you will need to: Obtain the physician’s order for transfer. Explain the reason for the transfer to the patient and family. Safety: Reconcile the patient’s list of medications, both prescription and over the counter, with any home medications and new health-care provider’s orders to ensure that nothing has been duplicated or omitted. The list is then reviewed with the nurse receiving the patient at transfer. (This is another safety requirement of The Joint Commission’s 2013 National Patient Safety Goals.) Gather the patient’s personal belongings, medications, nursing supplies, and complete chart.
