Many conditions can cause hemoglobinuria. In small-animalpractice, the most important causes areautoimmune hemolyticanemia(the animal’s immune system destroys RBCs) andincompatible blood transfusions. In horses,isoimmunehemolyticdisease of neonates(incompatible blood typesbetween mother and neonate cause hemolytic anemia in theneonate)is recognized as an important cause of hemoglobinuria.If the hemoglobin is in the urine because of lysis of intactcells that entered the filtrate,ghost RBCs(i.e., keratinized,denucleated cells with unstained, shadowy centers wherethe nuclei once were) should be present.The presence ofmyoglobinuriaindicates a muscle injury. Thiscondition may occur in a recumbent “downer cow”; prolongedpressure on one area of the body causes death (necrosis) ofthe muscle tissue that’s subjected to the animal’s heavy weight.This is one reason that it’s so important to note when a largeanimal is unable to rise. Other complications ensue when largeanimals are recumbent for a prolonged period, including bloatand respiratory and cardiovascular difficulties.Another type of muscle injury isexertional myopathy(tying-upsyndrome), a muscle dysfunction condition seen in horses.Other names for this condition areexertional rhabdomyolysisandazoturia. After exercise, the muscles of these horsesbecome hard and painful.The dipstick can give false-positive results for RBCs whencertain peroxidase-producing microbes are present. False-negative results occur with high levels of vitamin C in the urine.
