These surveys will be used to improve the resources found in the room. Also, the videos can be tailor-made as the patients and families request information found in the room. Multidisciplinary Team Team Member Role on the Team Patient Care Supervisor (PCS) This person is responsible for the idea of the resource room. The Education Nurse To help the team develop which core morbidities to videotape and how to obtain educational material. Marketing and Media To lead the videotaping and editing of the video information sessions. Also, to help market the new resource room to the public and staff. Patient Advocate The Patient Advocate has a relationship with the patients and makes rounds every day on all of the inpatients. Spreading the word of the resource room can be done as well as having patient and family input for information to include in the room. Charge Nurse on the Medical floor and Surgical floor These nurses have first-hand knowledge of the patients and what they are given at the time of discharge. They both can help include information that is not generally distributed on discharged or taught by the staff. They are both well known to the patient population

A patient and family member I would like to include a patient and family member to the team to have input as to what the public would like. They can also provide insight into the types of educational materials needed. Team Diversity Diversity of the team is due to the different members coming from different ethnic groups and social backgrounds as well as having different jobs within the hospital. The team will include nurses, marketing personnel, the patient advocate, and the educational nurse for the hospital. Each person has was chosen for what they can bring to the team. The nurses are well spoken and known to the patients and also known leaders within the organization. Marketing can help with the making of the videos and market the room to the public as well as in-house. The education nurse will be able to locate educational materials and sources of where and how to obtain them. The patient advocate will be able to encourage patients and family members to use the new room. He can also get ideas from the current patients about what type of educational materials they would like to see in the room. The patient and family member can add valuable input on the kinds of educational materials needed for the room.
