Risk Assessment Instrument
The health risk assessment tools compute risk of developing a disease, and the risk estimate is conveyed in numerical, text or visual formats which are used in a wide variety of contexts and for purposes including behavior counselling, screening for health issues and decision making (Grunfeld, McLaughlin, O'Brien, Straus, & Voruganti, 2015). Based on the assessments obtained from the patient that includes her chief complaint including associated manifestations, sociodemographic status, lifestyle practices, history, and physical exam, risks for specific conditions can be narrowed. Health risk assessment tool provides enhancement of preventive care by improving both individual health and population health through accurate estimation of risk for disease and improvement of the match between risk level and risk-management strategy which may also improve patient experiences by increasing activation and shared decision making, thereby incorporating their values and enhancing adherence to recommendations (Orlando & Wu, 2015). The pertinence of the health risk assessment tool will rely on the information obtained during the interview process; therefore, building a partnership with the patient through effective communication is vital to assess and address their health needs appropriately. Targeted Questions Once the initial rapport is built and communication barriers are addressed, focus questions will seek to gather as much as patient-centered information as possible. The targeted questions will be as follows: • How can I help you today? • How would you describe your health in general? • What medical problems have you had in the past and when did you have them? • How can you explain your lifestyle? • What aspects of your family of origin influence your health beliefs and behaviors? • What concerns do you have about working on improving towards better health practices? • What concerns worried you the most? • Is there anything else that you want to share with me? In general, the assessment, patient history and health questions are all part in participating in the awareness of the Asian ethnicity. The repour should encourage establishing a relationship that will lead to becoming the primary health care provider for this patient. Reference
