DIAPER RASH 3 Integumentary : Oozing and bleeding rash around the buttocks, thighs, and genitalia. She states that the baby cries when the diaper is changed. She also said that when she tries to touch her diaper area when applying petroleum jelly, the baby becomes uncomfortable and starts to cry. Milestone development :. At seven months, the baby can communicate nonverbally. The can communicate through facial expressions. Cries when she urinates or makes bowel movement because of the pain. Also cries when the diaper is changed Objectives Blood Pressure 90/60mmHg, heart rate 110 beats per minute, respiratory rate 35, Temperature 100.2 o F, Height 67cm, weight 17 pounds. Physical Examination General : Mild fever. No vomiting. No diarrhea or constipation. The baby is in discomfort when she changes the diaper. Crying because of pain when touched. Integumentary : No scaling. Presence of ulcerations around the buttocks, genitalia, and thighs. Presence of papules. Presence of vesicles. The mild eruption of some papules and vesicles.
