To review the service delivery implementation, discuss with the related person about the service delivery. Its quality and the feedback whether the service meets the client’s needs and requirementor not. Provide an individual support to make adjustment with the planning’s. Find out any problemthat is related to the delivered service and also record and report within the organisational procedures and discuss it and improve in such area that are not meet.A.3. During the process of facilitating individual service planning and delivery required documentation should be completed. It should be record clearly what are planned and decided. Formulate a report and other documentation and flow it within the related members to maintain accurate communication of that planning.
