The data gathered during the program will be analyzed thematically, which is ideal for extracting the primary themes and the guidelines to inform the change process from the wide array of data showing the relationships between adopting telehealth and the healthcare outcomes for the target group. For example, putting the evidence showing the effectiveness of telehealth for disease management drawn from participant engagement, patient observation, and researcher reflection will guide the change process, towards guaranteeing optimal outcomes. Finally, the key themes drawn from the data will highlight the relationships between the target themes, towards identifying the barriers that may affect the change process and normalizing the new service outlook into routine care service delivery. The data themes that emerged from the information gathered included that almost half of diabetic patients face the risk of poor disease management, which presents life-threatening risks (Alqarni et al., 2019). III. Improvement Plan Recommendation 1 : explore the quality of disease management for diabetic patients served by the hospital, during the Covid-19 pandemic. The data gathered showed that the disease management situation is worse for the patients, during the Covid-19 pandemic, a time when governments and healthcare organizations such as the World Health Organization recommend minimizing the physical contact between people (Napierała, Le´sniewska-Napierała & Burski, 2020). The reflection and observation from the healthcare team have shown that during the current health crisis, the hospital needs to reconsider its service delivery model, by augmenting it with a telehealth service delivery platform to

The service platform will be most useful as an addition to the medical services and disease management services, among other services, considering that it will improve the connection between service providers and consumers, while maintaining minimal physical contact (Napierała, Le´sniewska-Napierała & Burski, 2020). Background to the Decision on Practice Modification Before adopting the nursing-driven practice change, the nursing teams at the hospital relied on patient judgment skills in the Self-monitoring of blood glucose (SBMG) levels for the hospital’s diabetic patients (Type 2 diabetes). The traditional practice entailed providing SBMG guidelines to diabetic patients and relying on their skills and judgment for the management of the condition.
