For example, putting the evidence showing the effectiveness of telehealth for disease management drawn from participant engagement, patient observation, and researcher reflection will guide the change process, towards guaranteeing optimal outcomes. Finally, the key themes drawn from the data will highlight the relationships between the target themes, towards identifying the barriers that may affect the change process and normalizing the new service outlook into routine care service delivery. The data themes that emerged from the information gathered included that almost half of diabetic patients face the risk of poor disease management, which presents life-threatening risks (Alqarni et al., 2019).
