care situation? – I would apply the diabetes management assessment to patients who exhibit poor control of their diabetes. Documentation Assignments 1. Document your assessment findings for Red Yoder based on use of the SPICES assessment tool. – this was already completed and found the patient to have a problem with sleep disorders, incontinence, an evidence of falls, and skin breakdown. 2. Document assessment findings related to sleep and alcohol use for Red Yoder. – I did not thoroughly assess the patients sleep patterns and I should have completed this. I did assess the patient’s alcohol use and there was no evidence of a serious problem. He did admit to using alcohol to relax but it was not found that he had a problem with alcohol consumption. 3. Based on your assessment, document the nursing interventions related to wound care for Red Yoder. – full set of vitals taken, thorough pain assessment, assessment of wound with cleaning and change of dressing, performed alcohol use and screening assessment, determined ability for patient to care for wound and handle medication. 4. Following the assessment of Red Yoder, document the patient education provided that relates to the management of diabetes, with specific attention to care of the lower extremities. – education provided on diet and nutrition, diabetic foot sores, wound care, diabetes self-care, medication, and family support.
