Asthma – diagnosed as a child at age 2.5 years; last asthma attack in high school; Last breathing problem incident occurred 2 days ago due to cat allergy trigger. + history of hospitalizations – last was at age 16 for asthma. Prescribed Proventil inhaler to use 2 puffs to control asthma; however, reports using Proventil inhaler every 4 hours currently and needing more than 2 puffs, last use was this morning. Asthma triggered by cats, dust, and running up stairs. Gynecological and sexual history – Last LMP “about 3 weeks ago”, reports irregular periods and periods occur every 6 weeks to 2 months. Reports periods have a heavy flow for 4 to 5 days and needs to change extra absorbent tampon every 2 to 3 hours. Reports has heavy cramping and uses advil for pain relief. Reports no recent sexual activity, past sexual activity involves only men. Last sexual act “about 2 years ago”, total of 3 partners. No current use of oral contraception. Reports previous condom use and sexual activity without condom use. Reports knowledge gap in STI prevention. Last pap smear was 4 years ago. Denies previous pregnancies. Social History (Soc Hx) - Includes but not limited to occupation and major hobbies, family status, tobacco and alcohol use, and any other pertinent data. Include health promotion such as use seat belts all the time or working smoke detectors in the house. Occupation – supervisor Education level – currently work on Bachelor’s degree Diet – habitual diet soda drinking up to 4 per days, denies drinking coffee Substance use – past history of marijuana smoking,last use at age 20 or 21; + alcohol use, no more than 1 or 2 nights a week; Denies tobacco use. Environmental – denies secondhand smoke exposure Family History (Fam Hx) - Includes but not limited to illnesses with possible genetic predisposition, contagious or chronic illnesses. Reason for death of any deceased first degree relatives should be included. Include parents, grandparents, siblings, and children. Include grandchildren if pertinent. Mother – age 50, alive, hypertension, hyperlipidemia Father – diabetes Mellitus type II; died at age 58 in car accident Paternal grandfather – diabetes Mellitus type II; died at age 65 from colon cancer
