From the study, the research subjects found the pharmacist-led intervention very helpful as the patients were provided with the required needs and met the strategies of managing their care during the post-discharge period. Moreover, they reported that the discussion they had with the pharmacist was vital particularly how to prevent and manage side effects, and other self-medication procedures.Research MethodologiesPDSA (Plan, Do, Study, Act)The study focus on the communication in an outpatient clinic. Therefore, the study is based the assessment of 21 randomly selected outpatients to determine outpatient clinics provide a higher quality of outpatient care relative to traditional outpatient physicians who maintain hospital privileges with concurrent outpatient practices. This research is an original qualitative study and it will utilize the exploratory and descriptive research design.
