careful and avoid being given inaccurate information that can adversely impact continuity of care. Additionally, Snyderman et al. PATIENT-PROVIDER COMMUNICATION 11 reduce medication errors in a period of 30 days after the discharge date. Finding from the study are similar to the strategies posed by Snyderman et al. (2014) and Wang et al. (2014) as an aim of enhancing continuity of care. From the study, the research subjects found the pharmacist-led intervention very helpful as the patients were provided with the required needs and met the strategies of managing their care during the post-discharge period. Moreover, they reported that the discussion they had with the pharmacist was vital particularly how to prevent and manage side effects, and other self-medication procedures. Research Methodologies PDSA (Plan, Do, Study, Act) The study focus on the communication in an outpatient clinic. Therefore, the study is based the assessment of 21 randomly selected outpatients to determine outpatient clinics provide a higher quality of outpatient care relative to traditional outpatient physicians who maintain hospital privileges with concurrent outpatient practices. This research is an original qualitative study and it will utilize the exploratory and descriptive research design. Either a Medical Assistant, or myself the researcher will provide the data collection tool to the patients when they complete their appointment with the provider. Each questionnaire will have numerical order once completed the researcher will upload the data into a Google Form which will generate a spreadsheet. The method in which the data collection tool will be from a Liker Scale and a Statically Analysis will provide data that will be further analyzed the raw data with Statistics Solutions. (2016). Intellects Statistics (Version 1.01) [Online computer software]. Inclusion Criteria The research will involve 21 participants comprised of eight men and thirteen women whose age is between 17 to 45 years. The participants will be selected by using inclusion
