Moreover, in order for health care providers to deliver quality care, theirown well-being must also be taken care of. Bodenheimer and Sinsky (2014) suggestedthe use of Quadruple Aim, which adds another goal to the Triple Aim and focuses on thewelfare of the health care providers. They posited that health care should be a symbioticrelationship between the providers and the patients wherein both parties can benefit from,and that physicians who are unhappy with their practice, are more prone to prescribeimproper medications resulting in costly complications. When work conditions for thosewho deliver care are improved, the best care is provided and patient safety is ensured.Value and Relevance of Evidence Used in Patient Care PlanKoo et al. (2016) posited that data and metrics signify a critical part in establishing goalsand gauging the health of communities. Efficient organizational data-gathering methods areconsidered fundamental to health care quality, because the most crucial goal of PHIIs and thesuccess of its outcomes is dependent on a substantial evidence base. The methods andapproaches mentioned in the previous sections have not been perfect but have proven to beeffective in prior attempts for population health improvement.The Research and Development Corporation (RAND) is a non-profit organization thatpromotes the improvement of policy and decision-making by means of research and analysis(RAND Corporation, n.d.). They did a population-based study of interventions received bymilitary service members with a history of traumatic brain injury (TBI), utilizing methods thatcorrelated with the CCA and Triple Aim. They included demographics such as sex, age,

The results of their study guided the Defense andVeterans Brain Injury Center to implement PHIIs that helped enhance the quality of care andimprove health outcomes for service members diagnosed with TBI.Considered as one of the most effective health care systems in the world, the Mayo Clinichas been a trailblazer in the utilization of the PCMH model and has been recognized andendorsed by the National Center for Quality Assurance (NCQA). PCMHs have been shown to"improve quality and the patient experience, and increase staff satisfaction—while reducinghealth care costs" (NCQA, 2019). According to Adaji et al.
