Introduction : Purpose of the paper, synopsis of community, two CHN diagnoses
with supporting evidence, for example, observations, reports from Key
Informants and Statistical data. Remember to cite sources as appropriate.
The purpose of this paper is to bring awareness of obesity along with risk cardiovascular
disease to the community of Red Oak, Texas, 75154, population of 11,560 with 94 % urban and
6% rural (City-Data.com, 2014). Two CHN diagnoses noted within this community include risk
of cardiovascular disease among the community in 75154 related to observations of obesity,
diabetes diagnoses (may result in inability to access health care) and risk of obesity among the
community in 75154 related to observations of inactivity, reports of arthritis, observations of
multiple fast food places located in one central area. Multiple fast food restaurants are available
and easily accessible, and no community members were seen exercising outdoors. Two gyms
are available for joining for physical fitness, parking lots were seen with only a few cars at
different times of the day and week. EMS as per fireman MH, (MH, personal communication,
January 19, 2019) responds to numerous calls to those patients with obesity and/or diabetes.
School teachers, as per TG, (TG, personal communication, January 19, 2019), are noting an
increase in the number of teenagers with increased weight issues due to the availability of fast
food places near the school grounds. The adult diabetes rate in Ellis County is 8.3% compared
to the adult diabetes rate in Texas at 8.9% (City-Data.com, 2016). Adult obesity rate for Ellis
County is at 29.1% compared to adult obesity rate for Texas at 26.6% (City-Data.com, 2016). In
Red Oak, 20.2% of the population have been diagnosed with hypertension with 16.8%
diagnosed with hyperlipidemia (City-Data.com. 2016). The nearest hospital is located nine
miles away in Waxahachie. Cardiovascular along with obesity with underlying diabetes is
affecting the adults, and future adults, of this community. Access to education and need for
physical activity are warranted for this community to decrease the prevalence of obesity,
diabetes, and lack of physical activity.
Based on the 2 CHN Diagnoses you developed in Assignment 2 or modified,
select ONE problem and develop a CH Nursing intervention. Remember to cite
