I would like to know how I will add

the NSAID ibuprofen portion to the concept map. The med has the side effects that lead to HTN, Kidney failure, GI bleeding, etc. I am wondering if the patient should be taking something else for her pain. If she should, what is your recommendation? Thank you.

Mrs. Brown is a 65 yo woman who has a medical history (hx) of 3 chronic diseases.
Coronary artery disease (CAD), congestive heart failure (CHF), and hypertension (HTN)
as well as chronic pain. She is well known to your clinic because of her frequent visits
for adjustments of medication and exacerbations of her chronic diseases. She lives with
chronic pain due to low back injuries from her long time job as elementary school
cafeteria cook.
VS. BP 138/88: HR - 78; RR - 16; T -99.8. Her pain scale is on a level of 3-4/10.
5'4" and 180# (82 kg).
Current Medication List:
Allergies - NKDA; seasonal allergies to pollens
Lisinopril 10mg daily by mouth + 3
Metoprolol 25mg daily by mouth
Furosemide 40mg daily by mouth Lewis-
Detrol Long Acting (LA) 2mg daily by mouth
OTC Ibuprofen 200 mg 4 times a day as needed for pain
Social and Family Hx:
She recently retired from her cook position. Her husband died one year ago. She lives
with her oldest daughter and helps with watching her grandchildren while her daughter
and son in law work. She does not smoke or drink.
Items to ponder while concept mapping her case:
Describe how Mrs. Brown's pain is affecting her other health problems. Chronic pain is
a continuous stressor. Are there any social issues that may affect her problems and
outcomes.
The NSAID ibuprofen is the pain med of choice. How might this be affecting her heart
failure?
